Sleeve Gastrectomy

Sleeve gastrectomy is a bariatric surgery procedure that can be performed on patients with a body mass index of 35 and above, after the their evaluation based on the obesity eligibility criteria. Sleeve gastrectomy takes its name from the tubular postoperative anatomical shape of the stomach. Among the bariatric surgery procedures, sleeve gastrectomy is a surgical procedure more preferred by surgeons, performed through closed, i.e. laparoscopic methods with a lower complication rate in patients.

What is sleeve gastrectomy?

Sleeve gastrectomy is a type of bariatric surgery procedure based on the stomach reduction. At the end of sleeve gastrectomy, the stomach gains a tubular (banana shaped) appearance. Approximately 75-80% of the stomach is removed in sleeve gastrectomy. Removal of that portion of the stomach does not affect the progression of foods from the esophagus to the duodenum, and allows for maintaining the functionality. In sleeve gastrectomy, a large portion of the stomach is removed by means of certain automatic surgical instruments called staplers.

How is sleeve gastrectomy done?

The process of sleeve gastrectomy begins with the patient’s communication with the physician. Then the patient needs to get an appointment for examination. During the examination, the specialist physician who will perform the surgical application listens to the medical story of the patient, and patient can have a sleeve gastrectomy if the physician approves after performing the tests required in the examination process. Sleeve gastrectomy is performed with two methods. In the first method called ‘open surgery’ that has been used more widely in the past is performed by opening the abdominal region in the body. Today, the operation is performed through smaller incisions (about 1 centimeter long) by means of closed systems called ‘laparoscopy’.

How weight loss is achieved with sleeve gastrectomy?

In this procedure, also known as stomach reduction surgery, weight loss is achieved with two mechanisms. The first mechanism is based on a mechanical process, in which the stomach volume is reduced and then the remaining portion (about 150 ml) can be filled with a small amount of food. The amount of food intake and consequently the caloric intake are reduced with the reduced stomach size. The second considerable mechanism for weight loss is based on the weakened feeling of hunger resulted from the removal of the gastric tissue that produces Ghrelin, the “hunger hormone”.

The bariatric surgery procedure performed is colloquially known as an important obesity surgery that shows its effect in a short time.   • This procedures involves the reduction of the stomach volume, and consequently leads to an earlier feeling of satiety.
  • The procedure causes a considerably reduced feeling of hunger in individuals after surgery because it involves the removal of a large portion of the stomach, leading to less or trace amounts of Ghrelin (a substance causing the feeling of hunger) secretion.

What are the advantages of sleeve this operation over Gastric by-pass surgery?

  • The procedure is performed more easily.
  • When evaluated in terms of the duration of the operation, it takes shorter.
  • The post-operative resting period at the hospital, i.e the time spent at the hospital after surgery is shorter.
  • The rate of undesirable conditions (complications) is lower in sleeve gastrectomy.
  • In terms of practicality, it is a more appropriate procedure than gastric by-pass surgery, for human body, human anatomy, and human physiology.

What are the unfavorable effects of sleeve gastrectomy, and the conditions known as complications in the medical terminology?

  • Allergic and adverse reactions caused by drugs, which are used during the operation.
  • Transmission of diseases due to blood transfusion.
  • Different disease stories, heart attack, renal failure, urinary tract infection, pulmonary (lung) infection.
  • A low mortality rate of 0.2% (two per thousand).
  • Side effects that can develop in a way specific to the surgical procedure.
  • Clots formation in the leg veins (embolic risk)
  • Organ injury during surgery
  • Leakage from the stapler line (0,5-6,9%) after surgery, due to the stapler used during the operation.
